When I first started my career as a psychiatric nurse practitioner, I was eager, bright-eyed, and ready to put everything I had learned into practice. I wanted to help people heal—to truly restore their mental health and give them their lives back. But once I stepped into the world of community mental health, I quickly realized something unsettling: once you entered the system, you were in it for life.
No one ever sat me down during training to explain the reality of psychiatric treatment today. There was no “state of the union” address on the efficacy of our tools, no honest conversation about how few people actually achieve full remission. The goal seemed to be just stabilization—keeping people from crisis, avoiding hospitalizations, and managing symptoms just enough to get by. But what about actually getting better?
A lot of times, people sat down across from me with a huge list of medications they had tried—antidepressants, antipsychotics, mood stabilizers, and everything in between. They’d tell me why each one failed them: “This one made me gain 50 pounds,” “That one made me feel like a zombie,” “I had the worst brain fog,” or “It just didn’t work.” And then, almost every time, they’d say the same thing: “I just want something different.” That phrase stuck with me. People weren’t just looking for another medication to add to the list—they were looking for a whole new approach to their mental health care.
Looking Beyond the First Line: A Search for Better Care
I started digging deeper. In school, we are taught to follow treatment guidelines that prioritize older, generic medications. I spent hours reviewing clinical trials, emerging research, and real-world data to find better options—treatments that were more effective, faster-acting, and less disruptive to people’s lives.

Yet, the healthcare system barely addresses sleep, diet, or exercise when treating mental health. I started incorporating lifestyle-based interventions alongside medication:
